A Referring PhysicianReferrals
Doctors or gynaecologists who wish to refer a patient to First Steps Fertility must complete one of the referral forms below, and then fax it to 416.221.9020. Once the form is processed the date and time of the patient’s initial consultation will be sent to the referring physician’s office.
